  • 7 days prior started to have runny nose, fever, cough
  • Neck swelling started 3 days ago
  • Exam: temp 100.4, vitals otherwise normal
  • 1cm x1.5cm lymph node, mildly tender, mobile
  • Right anterior cervical chain
  • No skin changes or warmth
  • Rhinorrhea and congestion

15yo female with 5 days sore throat, odynophagia, fatigue, fever, swollen nodes
  • Exam: febrile to 101.3
  • Tired
  • Enlarged tonsils w/ exudates
  • Multiple 0.5-1.5cm nodes bilaterally
  • Posterior chain
  • Spleen is palpable
  • Rapid strep is negative 


What is the most likely diagnosis:

3yo male with red, swollen area of left neck
1 week prior: URI symptoms with fever and cough
°Small swollen area of left neck developed then
°Initial fever resolved
Now fever has returned x2 days
Swelling increasing, getting red, painful
Exam: temp 101.1
° Tender, red, indurated, non-fluctuant 2.5cm swelling in left anterior chain
°Oropharynx mildly red, normal tonsils

5yo male with a midline neck mass x a few months
Swelling initially got bigger but stable x 8 weeks
Exam: Afebrile, well appearing
°2cm, soft, midline neck mass
°No redness or tenderness
°Mass elevates with tongue protrusion
°No other masses or lymphadenopathy
Ultrasound: cystic midline mass, normal thyroid gland

What is the next best step?
